Medication Management
Offers psychiatric evaluation and ongoing medication monitoring for adults with behavioral health needs, ensuring safe, coordinated care alongside therapists, peer supports, and care teams.

 

Substance Use Disorder Outpatient Therapy
Supports individuals in addressing substance use and co-occurring behavioral health challenges through evidence-based therapy, including Cognitive Behavioral Therapy, to build coping skills and sustain recovery.

 

Substance Abuse Intensive Outpatient Program (SAIOP)
Provides structured, evidence-based treatment for individuals with substance use disorders, offering group and individual therapy multiple times per week while supporting continued engagement in daily responsibilities.

 

Substance Abuse Comprehensive Outpatient Treatment (SACOT)
Delivers a highly structured level of outpatient care with frequent clinical services, including therapy, care coordination, and skill-building for individuals needing intensive support without residential placement.

 

Peer Support Services
Connects individuals with Certified Peer Support Specialists who use lived experience to provide guidance, encouragement, and support in navigating recovery, accessing resources, and building independence.

 

Individual Support Services
Provides hands-on assistance with daily living activities for individuals with significant mental health needs, helping build skills for independence and improving overall functioning in the community.

 

Hospital Discharge Planning and Transition
Ensures continuity of care for individuals transitioning from inpatient, crisis, or detox settings by coordinating services and supporting stabilization in the community.

 

Assertive Engagement
Uses proactive, community-based outreach to connect individuals with needed services, reduce barriers to care, and promote consistent participation in treatment.

 

Transition Management (TCLI)
Supports individuals transitioning to community living by building skills for independence, managing mental health needs, and strengthening stability across life domains such as housing, health, and employment.

 

Community Support Team (CST)
Provides team-based, community-focused support for individuals with moderate behavioral health needs, helping reduce symptoms, access resources, and develop skills for independent living.

 

Assertive Community Treatment Team (ACTT)
Delivers intensive, 24/7 team-based support for individuals with severe and persistent mental illness, offering comprehensive services in the community to promote stability and reduce hospitalization.
